Director of Home Health Care Services

A new home health care agency called Sage Home Health is seeking an experienced, creative, and passionate individual to serve as our first Director of Home Care Services as we build a team in the Lebanon, PA area. A DAY IN THE LIFE AS A DIRECTOR OF HOME HEALTH

As the Director of Home Health, your duties will include Leadership, Financial, Patient Care, and Program Development related projects. You will work as part of a dynamic team that creatively meets goals and improves together. View the bullet points below to learn more about the core responsibilities of this position.

Leadership Duties 

  • Assists with the installation of a new home health organization in Central Pennsylvania, including seeking proper licenses from appropriate agencies.
  • Promotes, guides and participates in providing high-quality service.
  • Facilitates the development of home care clinical and administrative policies.
  • Develops, implements and reports organizational progress to the team.
  • Participates in selecting qualified team-members; evaluates and documents performance of employees; provides feedback and coaching; participates in corrective action process as appropriate.
  • Monitors the activities of home care to ensure compliance with federal, state and local laws and regulations.
  • Maintains a relationship with Skilled Nursing Facilities and other referral sources. Participates in professional and community organizations.
  • Promotes and maintains interdepartmental communications. Supports individual and team initiatives. Encourages creative and innovative thinking.
  • Fosters a culture of high-quality care/service and excellence in all aspects of operation.

Financial Responsibilities 

  • Monitors and analyzes revenues and expenses on a monthly basis with finance team. Maintains and monitors budget with the ultimate goal of providing the most cost-effective high-quality care possible.
  • Oversees electronic billing to ensure fiscal responsibility and proper reimbursement.
  • Ensures the most effective and efficient use of personnel and resources.

Patient Care Management 

  • Evaluates care through review of assessments and assures compliance with established standards of care in accordance with Federal and State standards.
  • Maintains, develops, and updates skills in order to implement high-quality care and service.
  • Acts as a resource for staff and provides leadership to promote a culture of patient safety and excellence.
  • Ensures excellence in care delivery provided by independent contractors by providing on-going and annual performance evaluations. Ensures standards of care are met by all personnel, including contracted staff.

Program Development 

  • Demonstrates responsibility for professional growth by establishing and attaining goals.
  • Encourages and provides an environment for professional growth and ongoing improvement of team performance. Communicates organizational goals and objectives to staff. Plans and conducts regular staff meetings.
  • Participates in the development of educational programs. Identifies educational and training needs of employees; ensures competency of staff is assessed, demonstrated and validated regularly.

QUALIFICATIONS FOR DIRECTOR OF HOME HEALTH

  • Required: Able to demonstrate excellent organizational, managerial and leadership abilities. Entrepreneurial skills in developing and spearheading new initiatives. Excellent communication, interpersonal, phone and customer service skills required. Basic computer skills. 
  • BSN preferred or related work experience. 
  • Preferred: Minimum two years experience in home care and/or hospice preferred. 
  • Current Pennsylvania RN license preferred.
